What case? I have a Chieftec case and my USB ports don't work but my USB headers do (6-in-1 reader/writer mounted in 3.5" slot).

it's just a cheap 50 dollar case i picked up for my GF (it's her comp)
I wish I had a Digi cam to take a pic for you guys... maybe in a day or so I'll borrow one

Refer back to Blade's diagram. I have a 8 wire ribbon cable coming from the front of the computer usb ports. The ID tags on the wire colours are Red +. White Port-, Green Port+, and Black - .The colours of the 8 wire ribbon are Black,Green, White, Red,Black,Green, White, Red. On his diagram Pin 1, Side 1 connect the Red wire. Pin 2, Side 1 the White wire. Pin 3, Side 1 connect the Green wire. Pin 4, Side 1 connect the Black wire. Then repeat the same procedure with Side 2. I plugged the device in and it was recognized right away. This sure makes life easier than groping around at the back of the computer.
